How to Register

1. Online– Internet registration is available 24 hours a day, seven days a week. You will receive an email confirmation once your registration has been received. If you are paying by credit card, we accept Visa, MasterCard, American Express and Discover. This is the easiest way to register.

2. Fax: Download the NSST 2016 Registration Form as a Word document. Note: credit card information must be provided. Instructions for faxing can be found on the form.

3. Mail: Download the NSST 2016 Registration Form as a Word document. Note: credit card information or check must be provided. Instructions for mailing can be found on the form.

4. Purchase order: If you need to pay by purchase order, please contact us.

*Faxed, mailed and purchase order registrations may take up to 10 business days to process.

REGISTRATION CANCELLATION POLICY

You may cancel your registration by sending a written cancellation request via e-mail, fax or mail to be received on or before April 1, 2016. A $50.00 processing fee will be deducted from your original registration rate. No refunds will be made after April 1, 2016.In the event that you cannot attend the conference, you may transfer or assign your paid registration to another participant by advising conference registration.

4 ADA credit hours
Advance registration required for Cessation Training and limited seats available.
